- Then, follow the instructions.
What you need: (to enroll a dependent)
A certified copy of a Birth Certificate for your children
A certified copy of a Marriage Certificate for your spouse
The last day to make changes is November 30. Don’t have changes? You don’t have to do anything!

Your benefit changes will start on January 1, 2026.
