In January, you received 2 new ID cards:

• Your Aetna medical ID card and
• Your CVS prescription drug ID card

Use your new ID cards, throw out the old ones!

Questions or need to order new/additional ID cards?

• Call Customer Service at 844-427-8501.
• Use the online Participant Portal.
• To download a photo of your CVS prescription drug ID card, use the CVS Caremark app or visit www.caremark.com.
  • Login and select “View ID Card.”
• To download a photo of your Aetna medical ID card, visit www.aetna.com.
  • Log in, click on the arrow by your name (top right corner), and select “ID cards.”
